Event Intake Information
Because each portrait session, event, and\or occasion is different, detailed information about your request is required to ensure all aspects of your occasion is satisfied. Please complete the from below and provide as much information about your requirements as possible and if the options you require are not listed in the form, please follow-up directly to provide specific details. You will be contacted within one business day to discuss and confirm your intake information and subsequent contract submission.
What to expect
Once you've completed the intake form, it is received by SIVAD Photography staff within one business day and reviewed for content details and logistics. Afterwards, you will receive a follow-up communication (based on the information provided) to confirm your input and develop a pricing format customized just for you.
Because each occasion is unique, we ask that you review your requests thoroughly to include all required elements of your occasion, any additions outside of what is listed and included in this form will be considered add-ons and assessed a requisite free accordingly.
Final note
Your intake form MUST be completed and reviewed before a pricing model can be provided.
Note: Completing this form does not constitute an event\session contract, rather, it is intended to provide detailed information about your event to develop a customized pricing model. All information must be entered into a contract form to confirm an event\session date.
